The Practice Transformation Specialist serves as a consultant to Advanced Medical Homes, Care Management Agencies, Clinically Integrated Networks, and provider practices in the Alliance Health region. The role improves quality of care through process improvement, data-informed care management, quality and utilization reporting, training, coaching, and support for alternative payment models. It is primarily remote, with occasional office attendance for business and team meetings, and requires a bachelor’s degree in nursing, social work, or a related human services field plus five years of post-degree healthcare or managed care experience. The salary is $69,592-$88,729 annually, with medical, dental, vision, life, long-term disability, retirement, paid time off, and flexible work benefits.
