Housing Works is hiring a Prevention Navigator to provide HIV prevention and care coordination in health centers and virtual settings. The role assesses clients, conducts HIV, Hep C, and STI testing, supports telehealth, connects clients to clinical and social services, maintains engagement, and coordinates outreach and prevention populations. It requires a bachelor’s degree in a health field or equivalent experience, strong communication and confidentiality skills, remote work capability, and willingness to work evenings and weekends as needed.
