Description
Housing Works is hiring a Prevention Navigator to provide outreach, screening, care coordination, health education, and navigation services to participants at risk for or living with substance use disorders, HIV, viral hepatitis, and other health conditions. The role supports referrals, program enrollment and retention, HIV and Hepatitis C services, continuity of care, documentation, and collaboration with multidisciplinary staff across two harm reduction program locations. Compensation is $21–$24 per hour, with healthcare plans, paid time off, and an educational benefit.
