Bloom Healthcare is hiring a Primary Care Nurse Practitioner or Physician Assistant in Loveland, Colorado to provide longitudinal, relationship-based primary care to homebound seniors and medically complex patients in their homes and communities. The role includes diagnosing and managing chronic and acute conditions, specialty visits, medication reconciliation, fall-risk and advance-care assessments, care coordination, and use of integrated data systems to reduce hospitalizations and readmissions. The position offers a Monday–Friday schedule, 8–12 patients per day, light call, weekends off, and compensation of $100,000–$125,000 annually, with 100% employer-paid health, dental, and vision coverage starting on day one.
