Bloom Healthcare is hiring a Primary Care Physician to provide longitudinal, relationship-based care to homebound seniors in the Dallas Fort Worth area. The physician will manage chronic and acute conditions, conduct specialty and comprehensive evaluations, coordinate post-discharge care, address social determinants of health, and collaborate with multidisciplinary teams. The role offers a Monday–Friday schedule, 10 patients per day, light call rotation, weekends off, and full health, dental, and vision coverage starting on day one, with annual compensation of $225,000–$260,000 plus potential performance bonuses.
