Description
Conviva is hiring a Senior Physician for its Middleburg center to provide outpatient primary care, continuity of care, health maintenance, disease prevention, chronic-condition management, screenings, and minor-injury treatment. The role coordinates specialty referrals, hospital and SNF services, durable medical equipment, and home health care; maintains medical records; participates in quality and care-team activities; and may join the local primary-care on-call program. Candidates need an unrestricted medical license or willingness to obtain one in Florida, an MD or DO degree, board certification or eligibility in Family Medicine or Internal Medicine, an unrestricted DEA license, and strong communication and EMR skills. The position offers a base salary of $255,000–$298,500 per year plus bonus, health insurance, malpractice insurance, relocation assistance, and other benefits.
