Action Wellness is hiring a Prison Linkage Program Case Manager to provide comprehensive case management and advocacy to HIV-positive individuals currently incarcerated or recently released from the Philadelphia County Prison system. The role conducts needs assessments, manages referrals, maintains care plans and documentation, coordinates medical, social, substance-use, and mental health services, supports hospital discharges and home care, and manages a caseload of 40–45 clients. The position is based at the Center City office and becomes hybrid remote after six months, with an annual pay range of $44,000–$49,000.
