Description
The Case Manager provides direct, person-centered case management to clients, including daily living skills training, entitlements assistance, behavioral health and medical assessments, referrals, crisis intervention, home visits, service planning, documentation, and rent and arrears monitoring. The role requires an associate degree, relevant client-population experience, and at least two years of mental or behavioral health experience, along with strong computer, organizational, communication, and cultural competence skills.
