Lives Lived Well is hiring a Program Coordinator for its Tokoko Integrated Recovery Pathways Program in Bundaberg. The role supports people transitioning from residential rehabilitation into community recovery by coordinating transitional accommodation, tenancy outcomes, housing and service access, recovery planning, relapse prevention, stakeholder referrals, and documentation. It is a fixed-term position for 38 hours per week until 30 June 2027, with regular travel throughout the Wide Bay region, and requires relevant qualifications or experience, recovery-oriented practice knowledge, strong communication and organisational skills, and a current Australian driver licence.
