Description
Community Care is hiring a full-time Program Supervisor for its Family Care Program, based in the Racine office with occasional travel to other offices, community events, and member visits. The role leads and mentors the Case Management team, performs case management activities, and supervises the day-to-day operations of the program. Applicants need a bachelor's degree in Nursing, Psychology, Social Work, or Human Services, current long-term care functional screener certification eligibility, and at least three years of case management experience with older adults or adults with intellectual or developmental disabilities; RN and leadership experience are preferred. The schedule is Monday through Friday from 8:00 AM to 4:30 PM, with health, dental, vision, paid time off, retirement, and other benefits.
