The Program Support Specialist of Family Connects New Jersey (FCNJ) Hunterdon County provides administrative, logistical, and operational support for a community health program. The role coordinates outreach and appointments with families, participates in bedside recruitment, supports mentoring and shadow visits, maintains appointment and follow-up records in Microsoft Office and Salesforce, documents outreach, responds to inquiries, conducts post-visit calls, and develops community and provider referral sources.
