Description
The Provider Performance Advisor is a field-based, market-focused role supporting provider practices in assigned Louisiana or neighboring-state markets. The position analyzes population health and performance data, establishes operational goals, monitors quality and utilization outcomes, identifies barriers, recommends improvements, and supports adoption of Equality Health’s value-based care and CareEmpower programs. The advisor also trains practices, represents the organization, and maintains feedback loops with the Practice Account Manager. The role requires a bachelor’s degree or equivalent healthcare experience, extensive local and regional travel, and at least three years of healthcare industry experience, including value-based care and population health management.
