Sutter Health is hiring a full-time Advanced Certified Hospice and Palliative Social Worker (ACHP-SW) preferred for PAMF-Palo Alto Medical Foundation ALD. The role provides integrative psychotherapy using expressive arts therapy, guided imagery, and evidence-based approaches for individuals, children, and families dealing with chronic or life-threatening illness. Responsibilities include mental health assessment, counseling, stress and grief support, behavioral change guidance, care coordination, treatment planning, interdisciplinary collaboration, and maintaining behavioral health and integrative medicine best practices. A master's degree in a mental health discipline and licensure as an LCSW, LMFT, or California psychologist are required, along with 2+ years of relevant experience.
