Northgate Health Care Facility is hiring a full-time Pulmonary Program Coordinator (RN) in North Tonawanda, New York. The role oversees daily pulmonary program operations, coordinates individualized care plans, monitors residents with respiratory and pulmonary conditions, collaborates with physicians and interdisciplinary teams, supports admissions and assessments, provides pulmonary care education, ensures regulatory and clinical compliance, and contributes to quality improvement and program development. An active New York State RN license is required, while skilled nursing, rehabilitation, pulmonary, respiratory, or subacute experience is preferred.
