Description
Fraser is hiring Case Managers to provide client-centered case management for individuals on CADI/BI and DD waivers, including connecting clients to community resources and advocating for their needs. The role serves adults and children, typically manages 35–43 clients, and requires travel to meet clients 2–3 times per week. Candidates must reside in Minnesota or the Twin Cities Metro Area, hold a relevant bachelor's degree, have the specified experience or willingness to complete the Relias I/DD course, and pass a DHS background study.
