Wiltshire Council is hiring a Reablement Support Worker to provide home-based support to individuals after hospital discharge, helping them achieve personal goals and co-develop reablement plans with occupational therapists. The role requires rota availability between 7am and 10pm, 365 days a year, customer travel, a valid UK driving licence, and access to a vehicle unless an alternative arrangement is agreed. It pays £13.92–£14.36 per hour plus 10% unsocial hours for qualifying shifts, runs for 37 hours per week, and offers NVQ apprenticeships and upskilling.
