Wiltshire Council is hiring Reablement Support Workers to provide home-based support to individuals after hospital discharge, helping them achieve personal goals and work with occupational therapists on personalised reablement plans. The role requires rota availability between 7am and 10pm, 365 days a year, travel for customer visits, a valid UK driving licence, and access to a vehicle unless an alternative arrangement is agreed. Applicants are continuously reviewed and interviewed weekly, with applications accepted until sufficient applications are received.
