Description
The Registered Nurse - Health Services provides nursing care to adults and children with physical health conditions, mental illness, substance use disorders, and developmental disabilities in community-based settings, outpatient clinics, and other assigned locations. The role includes health teaching, medication administration, care coordination, transition planning, documentation, quality improvement, and collaboration with consumers, families, and interdisciplinary teams. It reports to the Supervisor of Business Practices-Health Home and the Chief of Health Services and Integrated Care, and may provide clinical coaching and mentorship to other agency healthcare providers.
