The Registered Nurse (RN) Case Manager serves as a patient care guide for members facing catastrophic illness, injury, or acute and chronic medical conditions. The role identifies members for case management and chronic condition management, applies nursing processes to coordinate integrated health services, provides education and guidance, monitors care quality and outcomes, documents activities, and collaborates with healthcare teams. The position is based at the Dublin, Ohio campus with rotating work-from-home days and no weekends or holidays.
