Crown &Seasons Hospice is hiring an RN Case Manager to serve as a clinical leader and patient advocate, coordinating care across homes, nursing facilities, and assisted living communities. The role includes comprehensive patient assessments, individualized care planning, interdisciplinary collaboration, discharge planning, care-plan monitoring, patient and caregiver education, documentation, and backup on-call support. Applicants need a current Missouri Registered Nurse license, strong patient assessment and case management experience, and experience in geriatrics, long-term care, home health, or hospice is highly valued. The preferred work areas are Fredricktown, Advance, or Marble Hill, with a salary range of $78,000–$81,000 DOE.
