Roper St. Francis Healthcare is hiring two care-manager roles: a Registered Nurse (RN) Care Manager and a Master Social Worker (MSW) Care Manager. Both work 36 scheduled weekly hours on a weekend-only day shift from Saturday through Monday, 7:00 a.m. to 7:30 p.m., in a general office environment. The RN Care Manager coordinates patient care, discharge planning, care transitions, documentation, utilization management, and multidisciplinary collaboration, while the MSW Care Manager provides social services, discharge planning, community-resource access, and clinical supervision. Both roles require BLS, RN or social-worker licensure, and at least one year of clinical experience; the RN role requires a Bachelor of Science in Nursing, while the MSW role requires a Bachelor of Social Work.
