Blue Ridge Care is hiring a Registered Nurse (RN) for its PACE Program to provide primarily field-based, in-home nursing care to older adults in homes, assisted living facilities, and community settings. The role includes clinical assessments, medication management, wound care, injections, phlebotomy, chronic disease monitoring, care-plan development, caregiver education, care transitions, safety assessments, interdisciplinary collaboration, documentation, urgent clinical response, and on-call participation. Candidates must hold current Virginia RN licensure, BLS/CPR certification, at least one year of experience with frail or elderly populations, medical clearance, a valid driver’s license, proof of insurance, and reliable transportation.
