Description

Springfield Clinic is hiring a Remote Patient Monitoring Care Navigator to coordinate Chronic Care Management and Remote Patient Monitoring programs for older adults with chronic health conditions. The role tracks remote-monitoring data, communicates with patients and clinical teams, provides health education, documents care in EHR systems, coordinates appointments and referrals, escalates risks, and supports patient engagement. Candidates must be qualified as an Illinois-licensed LPN or credentialed medical assistant, and must obtain CPR certification under American Heart Association guidelines within 30 days of hire.

Additional Information

Preferred Qualifications: Current Illinois LPN licensure.
Current RMA, CMA, or NCMA credential.
Registration or certification through an accepted medical assistant credentialing organization.
