Guidehealth is hiring a remote Care Manager to support patients and families navigating complex and chronic health journeys. The role creates personalized care plans, coordinates care across providers and settings, educates and advocates for patients, collaborates with interdisciplinary teams, and uses data to measure impact. Candidates need at least five years of Registered Nurse experience, an active RN license, care-management and chronic-condition expertise, motivational interviewing and patient-education skills, remote-work independence, and comfort with multiple EMR systems. The position offers a fully remote work arrangement, medical, dental, and vision benefits, and a base salary of $78,000 to $83,000 per year.
