Description
The Residential Case Manager supports individuals in residential care settings by assessing physical, emotional, and social needs; developing individualized care plans; coordinating healthcare, counseling, education, and community services; advocating for residents; maintaining case documentation; providing crisis intervention; and collaborating with families, care teams, and external agencies. The role requires at least two years of case management or residential care experience, strong communication and organizational skills, and knowledge of social service systems. Preferred qualifications include a bachelor’s or master’s degree in social work, experience with veterans or individuals with mental health, substance use, or developmental disabilities, familiarity with electronic case management systems, and Spanish bilingual ability.
