Description
This residential support role combines educational, nursing, and social work to support people with physical disabilities and complex support needs in an eight-person residential group. Responsibilities include individual care and nursing support, personal care, medical and therapeutic accompaniment, communication and social participation, daily living assistance, leisure and participation planning, outings and vacation programs, digital media support, person-centered offers, change-process support, family and multidisciplinary collaboration, documentation, and individual support planning. The position works on shift duty, may include night care, and offers structured onboarding for applicants without prior inclusion-support experience.
