Ohio’s Hospice is hiring a field-based RN Admission Care Liaison for its South Region, serving the greater Miami Valley and surrounding communities. The role conducts hospice admissions, assesses patient needs, coordinates hospice services, educates patients and families, documents care in the electronic medical record, and maintains relationships with healthcare partners. The schedule is 11:00 a.m. to 9:30 p.m. with every third weekend rotation and 1–2 holidays per year, and the position requires travel throughout the service area. Required qualifications include an accredited nursing program, an active Ohio RN license, CPR certification, electronic medical record proficiency, a valid driver’s license, reliable transportation, automobile insurance, and successful background and drug screening. Preferred qualifications include an ASN or BSN, two years of acute care nursing experience, and CHPN certification.
