Ohio’s Hospice is hiring a part-time Admission Care Liaison (Registered Nurse) in Newark, Ohio, working 10:00 AM–10:00 PM every third weekend plus rotating holidays. The role supports patients and families transitioning to hospice care by conducting assessments, evaluating patients in hospitals, long-term care, assisted living, and home settings, educating families, coordinating care, communicating clinical findings, supporting eligibility and certification, and assisting with discharge planning. Candidates must be graduates of an accredited nursing school, hold a current Ohio RN license, have CPR certification, and meet driving and background-screening requirements; acute care experience and CHPN certification are preferred.
