Description
The Care Coordinator Nurse (RN) supports a Medical Home Care Team by coordinating patient care, reviewing medical records, scheduling referrals and treatments, developing care plans, providing patient education, conducting follow-up calls, documenting interactions in electronic medical records, and supporting clinical team members. The role requires active RN licensure and valid BLS certification; two years of health-related care supervisory experience is preferred but not required.
