PACE North is hiring a full-time, in-person RN Care Manager to serve as an interdisciplinary care decision-maker for adults 55 and older. The role includes developing care plans, conducting health assessments, coordinating supplies and services, visiting participant homes, participating in on-call rotations, leading hospital-to-home discharge planning, and providing education, advocacy, and leadership. Candidates must graduate from an accredited nursing program, hold a current Michigan RN license, have a valid driver’s license, and demonstrate strong critical thinking and collaborative patient-care skills.
