VIVA HEALTH is hiring an RN, Care Manager in Birmingham, Alabama to evaluate members' needs, guide families toward appropriate wellness resources, and manage physical, environmental, and psychosocial health issues through collaboration with a multidisciplinary team. The role primarily performs fieldwork outside the office, travels within the service area, and offers work-from-home opportunities with occasional on-site work. Required qualifications include an accredited nursing program, three years of clinical nursing experience, an Alabama RN license, a valid driver's license, and clinical assessment skills for chronic disease management in adults.
