Description
VHS Hospice is hiring a full-time RN Case Manager to coordinate and manage care for patients with chronic, acute, or complex medical conditions. The role includes conducting patient assessments, developing and evaluating care plans, advocating for residents, communicating with families and healthcare providers, coordinating appointments and referrals, and educating patients and families. An RN Associate’s Degree in Nursing is required, while a BSN and prior case management, utilization review, or direct patient care experience are preferred. The position is available in Newport News, Hampton, Kilmarnock, Gloucester, Yorktown, and Southside, Virginia, pays $35-$45 per hour, and includes health, vision, dental, retirement, and other benefits.
