Description
The RN Case Manager manages comprehensive home health services for patients in their place of residence, including assessments, care-plan development, goal setting, interventions, physician-order compliance, medication reconciliation, triage, care coordination, patient and family education, advocacy, and on-call support. The role also supervises home health aides, maintains accurate documentation, and participates in quality and safety activities. It requires an associate’s degree in nursing, current Massachusetts Registered Nurse licensure, CPR certification, and a valid driver’s license; one to two years of healthcare experience is required, while home-health experience is preferred.
