Description
The RN Case Manager coordinates, monitors, and evaluates patient care across the healthcare continuum, develops case management plans, supports discharge planning, collaborates with physicians, patients, families, staff, and insurance payers, and documents care activities. The role requires an active Colorado RN license, a bachelor's degree in nursing, at least three years of clinical hospital experience, and BLS certification; hospital case management experience is preferred. The position offers 36 guaranteed hours per week, including weekend coverage, and is described as a quick-start opportunity.
