Description
The RN Case Manager provides direct nursing care and routine medical procedures for ICF/IRA residents and staff, coordinates acute and ongoing health services, maintains nursing assessments and care plans, documents clinical findings, and trains direct-care staff in health, medication, tube-feeding, diabetic, and seizure-related care. The role supervises LPNs, AMAPs, and Direct Support Professionals, participates in interdisciplinary teams, and requires an active New York State RN license, electronic health record proficiency, and strong communication and independent-judgment skills. A bachelor’s degree and at least three years of clinical experience in a hospital or primary care setting are preferred, along with experience with the intellectually disabled population and managerial experience.
