Description
The RN Case Manager provides clinical nursing support and coordinates patient care under the direction of the Director of Nursing and providers. Responsibilities include patient advocacy, intake and referrals, nursing visits, point-of-care testing, medication administration, triage, care planning, follow-up, crisis intervention, patient education, infection control, recordkeeping, and supervising clinical team members. The role requires a Bachelor of Science or Associate degree in Nursing, a current Louisiana RN license, 3–5 years of case management experience, and current Basic Life Support certification; infectious disease and behavioral health experience is preferred. Local or out-of-state travel may be required.
