Description
The RN Case Manager at Klamath Tribal Health and Family Services works within the Wellness Center to coordinate care for patients with chronic conditions or complex needs, provide education and motivational support, manage disease registries and preventive care, and support clinic operations, triage, quality improvement, and nursing mentorship. The role requires a current RN license, ACLS certification, basic computer and EHR skills, and compliance with Tribal, immunization, and background requirements; a BSN and three years of relevant clinic experience are preferred.
