Description
The RN, Case Manager provides direct clinical, palliative, and supportive care to patients and families, assesses home safety and care needs, coordinates individualized Plans of Care with an interdisciplinary team, obtains physician orders, documents care, and supervises Community Health Workers and Home Health Aides as needed. The role requires an active RN license, BLS certification, EMR experience, reliable transportation, and the ability to perform physical duties such as lifting and bending. Compensation is $61,860.66 to $96,657.28.
