Description
Mountain Valley Hospice is seeking a Registered Nurse Case Manager to provide direct nursing and supportive care to terminally ill patients in patients’ homes, skilled nursing facilities, and assisted living facilities. The role collaborates with multidisciplinary providers, guides Certified Nursing Assistants, monitors patient conditions, and documents changes. The position offers Monday–Friday 8-hour days or four 10-hour days, with flexibility to start between 7 AM and 9 AM and a weekend rotation every eighth weekend. A valid Registered Nurse license, strong interpersonal and communication skills, and the ability to work independently and collaboratively are required.
