Mountain Valley Hospice is hiring a Registered Nurse Case Manager in Martinsville, Virginia, to provide direct nursing and supportive care to terminally ill patients in patients’ homes, skilled nursing facilities, and assisted living facilities. The role involves collaborating with multidisciplinary teams, guiding Certified Nursing Assistants, monitoring and documenting patient condition changes, and communicating updates to the care team. The schedule offers Monday–Friday 8-hour days or four 10-hour days, with flexibility to start between 7 AM and 9 AM and a weekend rotation every eighth weekend. Applicants must hold a valid Registered Nurse license and demonstrate strong interpersonal, communication, compassionate, independent, and team-based skills.
