Description
The RN Complex Case Manager provides complex clinical case management for high-risk patients in an ambulatory setting. The role coordinates care, develops and monitors individualized care plans, assesses and monitors utilization patterns, supports interdisciplinary teams, provides follow-up during care transitions, and contributes to quality improvement and population health. The position requires Nevada RN licensure, current CPR or BLS certification, an accredited nursing school background, at least three years of clinical experience, familiarity with electronic health records, and at least two years of complex case management experience.
