Description
The RN Coordinator – Home Visiting Program provides longitudinal care management and coordination for medically complex, homebound, and high-risk patients. The role includes telephonic and virtual assessments, clinical triage, care planning, medication reconciliation, post-discharge outreach, care transitions, referrals, interdisciplinary collaboration, home visits, documentation, and support for social determinants of health. It is a hybrid position requiring an active RN license, bilingual Spanish, at least two to three years of clinical experience, and familiarity with care coordination workflows, telehealth, and electronic health records.
