Capital City Gardens is hiring a full-time RN MDS Coordinator to manage the RAI process and ensure timely, complete MDS, CAA, and interdisciplinary care plan documentation. The role involves coordinating assessments, supervising MDS input and state transmission, facilitating care plan conferences and QM meetings, and managing the care planning process. Candidates must have a nursing degree, an active RN or LPN license, CPR, and at least 2 years of long-term care MDS Coordinator experience, with strong knowledge of case-mix, Medicare PPS, Medicaid reimbursement, and regulatory standards.
