Description
The MDS/Care Planning Coordinator will coordinate the planning, scheduling, completion, certification, and submission of Minimum Data Set assessments and care plans for residents at Tsali Care Center. The role oversees compliance with federal, state, and tribal requirements; maintains medical records and data quality; prepares reports; and collaborates with residents, families, physicians, case managers, and other health professionals. It also includes RN floor duties such as participating in quality improvement projects and addressing nursing care problems. The position requires a North Carolina Registered Nurse license, nursing school graduation, at least one year of nursing experience, six months of MDS/Care Planning and long-term care experience or three years of long-term care experience, a valid North Carolina driver’s license, and proficiency with Point Click Care and Microsoft Office.
