PruittHealth is hiring a primarily remote Registered Nurse Case Manager to oversee clinical care for a community-based long-term care population in its Premier Dual Eligible Special Needs Plan. The role coordinates care across community providers, social support programs, hospitals, and other care settings; conducts assessments and care planning; supports medication therapy and benefits preservation; facilitates interdisciplinary care; and documents case management activities. The position requires an Advanced Nursing Diploma or nursing degree, at least two years of direct clinical care experience, a Georgia unrestricted RN license, current CPR certification, and strong remote case management, communication, and organizational skills.
