RN, Supervisor at Activate Care

Activate Care is hiring a full-time RN Supervisor to lead a distributed team of RN Assessors conducting in-person health assessments for dually eligible members across Massachusetts. The role combines clinical quality and audit, team leadership and development, caseload and route planning, KPI reporting, compliance, and stakeholder partnership. It requires an active Massachusetts RN or higher licensure, a BSN, at least three years of RN experience including home health or community-based assessment work, supervisory experience, and the ability to lead a remote, field-based team. The position is field-based and hybrid, requires regular travel, and offers standard benefits and a sign-on bonus.
