Description
Pioneer Valley Hospice & Palliative Care is hiring a full-time Weekend Coordinator Registered Nurse for a 32-hour Friday, Saturday, and Sunday 8 a.m. to 8 p.m. schedule. The role oversees hospice patient care on weekends, including assessments, plans of care, medication administration, symptom management, referrals, EHR documentation, interdisciplinary coordination, and supervision of hospice aides and LPNs. The position is primarily based in the Greenfield office with travel to patient and family homes, and requires at least three years of health care experience, hospice or home-care experience, an Associate’s Degree in Nursing, current Massachusetts RN licensure, and CPR certification.
