Description
The Home Health Scheduler facilitates daily scheduling of home visits for field staff and coordinates patient care across RNs, LVNs, PTs, PTAs, OTs, COTAs, STs, HHAs, and MSWs. The role assigns visits by geographic area, tracks staff loads and skill mix, projects schedules from care plans, handles reassignments and new referrals, verifies authorizations, maintains EMR records, and supports regulatory compliance. The position requires a high school diploma, computer proficiency, a licensed driver’s status, and preferred health-care and administrative experience.
