CHD's West Springfield clinic is hiring a Bachelors-level Case Worker to serve youth at West Springfield Middle School who may be at risk for substance use or violent behavior. The role collaborates with a Master's-level Clinician to implement treatment plans, supports youth and families with emotional and mental health needs, coordinates care with community providers, develops service plans, facilitates small groups, provides recreational and community activities, and offers homework help. The position requires a bachelor's degree in a related field, knowledge of mental illness, behavioral modifications, and parenting education, basic computer skills, a valid driver's license, and availability Monday through Friday from 8 a.m. to 4 p.m. The annual salary is $45,000, with dental, health, and life insurance and paid time off.
