Description
The School Based Support Specialist provides intensive outpatient case management and behavioral health support to youth in a school setting, delivering individualized interventions aligned with treatment plans, coordinating care with schools, families, and community providers, and supporting recovery, resilience, and academic success. The role includes crisis intervention, outreach, documentation, quality assurance, and compliance work, with services delivered primarily on school premises during school hours and additional home/community support as needed.
